Test Relocation Form

Fill out this form if you’re approved for testing accommodations and need to take a test with SAS.

Student Accessibility Services:
If you qualify for accommodations because of a disability, please submit your accommodation letter from Student Accessibility Services to your faculty member in a timely manner so that your needs can be addressed.  Student Accessibility Services determines accommodations based on documented disabilities in the academic environment.  Information on requesting accommodations is located on the Office of Student Accessibility Services website.
